Full Circle Health Network’s providers are pioneers in the principles of trauma-informed care, wraparound, and person-centered service delivery.

These community-based organizations are embedded in their communities and have decades of experience serving individuals with complex needs across multiple systems, such as child welfare, justice, and specialty mental health.
Here we spotlight two of the more than 100 FCHN providers operating statewide who also are pioneering how to integrate Enhanced Care Management (ECM) into their multitude of programs to better service children, youth and families.
Vista Del Mar Child and Family Services and Victor Community Support Services are two examples of FCHN network providers that are working diligently to address unjust inequities through trauma-informed approaches to care. These providers hire people from the communities they serve and prioritize delivering culturally congruent care.
Vista Del Mar has served vulnerable children and families across Los Angeles for years, offering everything from mental health treatment and residential care to foster care, adoption, and autism services.

In 2025, they took their model a step further by launching a ECM through h Full Circle and they are steadily growing their ECM enrollment.
Standing up a new program means new staff, new workflows, and complex administrative requirements. Full Circle provided the infrastructure, managed care expertise, and hands-on guidance to handle the operational side, freeing Vista Del Mar's team to stay focused on clients.
The result is a care model where every member of a client's multidisciplinary team - primary care, mental health, and beyond - is working in coordination.
Looking ahead, Vista Del Mar plans to expand ECM services, deepen Community Supports integration, and continue building a sustainable, holistic model of care across LA County.

Victor operates across many counties in California. Despite being an organization with more than 1,000 employees, standing up a new program with new payers - managed care plans - posted implementation challenges.
